Who Wins

Pre-6, you lose the 2v2.
Nautilus's hook into Nami's bubble is a two-CC chain that kills you before your Q stacks matter, and Karthus's Q poke punishes you every time you step up to farm.
Post-6, your R fundamentally changes the fight — your W becomes a laser with a heavy slow that denies Karthus's positioning, and your sustained DPS in Transcendent State outscales Karthus's burst if you survive the initial CC.

Let Nautilus hook first to burn Nami's E or force her to react, then activate your R immediately and use your upgraded W laser to slow Karthus in place — he has no mobility and dies to your stacked on-hit damage in extended fights.
Never trade without your R unless Nautilus has already landed his hook and the CC chain is guaranteed.

Push the wave consistently using your Q's splash damage to shove minions, which forces Karthus to either take poke or lose CS — he cannot safely walk up to last-hit under your Q pressure.
Avoid freezing; a pushed wave gives you safety from Nautilus's hook range and reduces the window where Nami can bubble you in the open.

Do not walk forward to trade without knowing where Nami's E is — her empowered auto after bubble is a full CC chain that sets up Nautilus's passive and kills you before your Q ramps up.
Your immobility pre-6 is a hard punish target, so respect the hook angle and never stand in a position where both Nautilus and Nami can reach you simultaneously.
