Who Wins

Pre-6, you win the 2v2 if you land your E — the bubble into Nautilus hook chain is a guaranteed kill on Yunara who has no escape, and Karthus's Q poke compounds the pressure.
Post-6, Yunara's R dash makes your bubble harder to land and she can reposition over walls, so the window to all-in narrows significantly.

Use your E on Yunara when she steps up to farm, then immediately follow with your W heal on Karthus to sustain through any retaliatory poke — this forces Yunara to either back off or take the full CC chain.
Do not use your bubble reactively; save it for when Nautilus is already engaging so the double CC guarantees the kill.

You should not roam aggressively — Karthus has no mobility and is vulnerable to Nautilus dives without your peel, and losing Karthus to a dive while you're roaming gives the enemy bot lane a free kill and tempo.
If you do roam, only do so after a successful kill in lane when Karthus is safe under tower with a pushed wave.

Do not waste your W heal on Karthus when he takes minor poke — save it for when he's in a full trade or being dove, because running out of mana from over-healing means you cannot bubble when Nautilus needs to engage.
Your mana is your most limited resource in this lane and spending it reactively on small heals loses you the kill setup.
