Pre-6, you lose the 2v2 if Blitzcrank lands Q at level 2 because you have no dash and your W root requires setup time that the hook combo doesn't give you — however, if Thresh plays aggressively with E+Q to zone Blitzcrank, you can win short trades with your superior range and passive damage.
Post-6, the matchup becomes more even because your R can interrupt or punish a Blitzcrank engage from range, and Thresh's R gives you a reliable setup that doesn't require you to be in hook range.

Your best trading pattern is to use Thresh's E slow to kite Blitzcrank's W approach, then land W root on MF or Blitzcrank to set up Thresh's Q for a controlled all-in — never let Blitzcrank close the gap freely because your fourth shot and W are your only real damage tools and both require setup.
Use your range advantage to poke MF down before committing to any trade, as she has no sustain and will be forced to back or play passively.

You want to slow-push the wave by letting Thresh poke with Q and E while you last-hit safely, building a large wave that crashes into their tower and forces MF and Blitzcrank to choose between farming and engaging — a frozen wave near your tower is also viable since Blitzcrank cannot safely hook under tower without risking tower aggro.
Avoid pushing aggressively because it puts you in Blitzcrank's hook range in the open.

Do not walk up to last-hit without checking Blitzcrank's W status — if his W is active and he is moving toward you, you are already in danger and your W root will not save you if he closes the gap before you can cast it.
Never position in a straight line with Thresh, as MF's R will shred both of you simultaneously if Blitzcrank lands a hook and you are grouped.
