Who Wins

Pre-6, you lose the 2v2 if Leona hits her E+Q on you — her level 2 spike is faster than yours and Sivir's damage on a stunned target is lethal.
However, if Blitzcrank hooks Sivir first, you can Q onto her freely and the kill is yours; the lane is coin-flip pre-6 depending on who engages first.
Post-6, Leona's R gives them a reliable initiation that your kit cannot easily escape, so you need to be ahead or play for picks rather than straight 2v2s.

Your best pattern is to wait for Blitzcrank to land a hook on Sivir, then Q stack onto her immediately to proc your passive stun — do not engage independently because Leona will chain CC you.
If Sivir uses W to block the hook, back off and reset; do not force a trade into a shielded Sivir with Leona ready to engage.

You want to freeze the wave near your tower when possible, forcing Sivir to walk up into Blitzcrank hook range to last-hit.
Avoid shoving the wave into their tower, which gives Leona free engage angles and removes Blitzcrank's hook threat as a defensive tool.

Do not use your E to try to disengage from Leona's E+Q combo — your E does not interrupt her stun and you will waste it.
Also avoid stacking Q onto minions aggressively when Leona is at full health with her E ready, as she can engage mid-Q chain and you will be stunned with no escape.
