Who Wins

Pre-6, you and Zilean win the 2v2 because your level 2 spike with double Q bombs into your passive burst is one of the strongest all-ins in the bot lane, and Milio's healing cannot compensate for the burst damage in that window.
Post-6, the dynamic shifts against you because Milio's R directly counters your R channel and Zilean's R, removing both of your ultimates' value simultaneously.

Your best trading pattern is to let Zilean land a double bomb stun first, then immediately walk up to proc your passive on Smolder and follow with Q for the double-up bounce, maximizing burst before Milio can react with W or R.
Avoid extended poke wars because Milio's W sustain and range boost on Smolder will slowly win attrition if you don't convert trades into kills.

Shove the wave hard with your W to crash it and create roam opportunities for Zilean, as Smolder is slow to clear early and a frozen wave under your tower is dangerous for him.
Use cannon minion waves specifically to threaten double-up Q poke onto Smolder, as the bounce damage is significant and forces Milio to spend his healing reactively.

Do not begin your R channel unless Milio's R is confirmed to be on cooldown — casting R into an available Milio R is a complete waste of your ultimate and will likely result in you being the one who dies during the channel animation.
