Who Wins

Pre-6, you lose the 2v2 because Lillia's W slow and Q chip damage reduce your mobility advantage, and Twitch's poison stacks up even through your movement.
Post-6, you still do not win a clean 2v2 because Twitch R gives him massive DPS and Lillia R can separate you from Soraka, but you can survive and disengage better with your E.

Your best trading pattern is to use E to reposition behind minions after poking with your Q passive bounces, forcing Twitch to either stealth or take poke.
Never commit to a sustained trade because Twitch's poison and Lillia's W will out-damage Soraka's healing over a long exchange.

Push the wave fast with your W to deny Twitch passive stacking and to avoid slow-wave scenarios where Lillia can freely poke you.
A fast push also lets you back safely and denies Twitch the ability to stealth into a favorable engage on a frozen wave.

Do not use E aggressively into Lillia's W zone thinking your mobility will save you — Lillia's W slow stacks with Twitch poison and you will be kited in place.
Save E as a defensive escape tool, not an engage tool in this matchup.
