Pre-6, you have a meaningful advantage at level 1 and 2 through Q poke range and passive blob sustain, allowing you to chip Mordekaiser down before he can stack his passive on you — however, at level 3 Mordekaiser's E pull combined with passive stacking makes standing near him dangerous, so respect his all-in threat.
Post-6, you fundamentally lose the 1v1 because Mordekaiser's R removes your E escape, your passive blob survival mechanic, and your ability to call for help, while he steals a percentage of your stats — avoid getting caught by R at all costs.

Your best trading pattern is to use Q at max range to poke Mordekaiser repeatedly without letting him stack his passive on you, then disengage using your passive blobs to sustain back up — never commit to a full melee trade where his E, passive aura, and W sustain will outlast you.
If you need to engage, do it with full HP and only when you can guarantee a short burst trade followed by an immediate exit before his passive aura deals significant damage.

Prioritize a slow push or freeze near your side of the lane to deny Mordekaiser from freely walking up and stacking passive on you, since he needs to be in melee range to apply pressure and a freeze forces him into unfavorable trades to break it.
Use your wave advantage to roam for objectives and skirmishes where your E engage and team-fight utility far outclass Mordekaiser's, as he has no meaningful response to you impacting the map while he is stuck in a frozen lane.

Never use your E to engage onto Mordekaiser in a situation where he can immediately R you, because you will be trapped in the Death Realm with no escape, no blob regeneration, and reduced stats — his R is specifically designed to punish exactly the kind of all-in dive that defines your kit.
Additionally, do not ignore your passive blobs when they fall off during a trade; losing them means losing your only safety net against Mordekaiser's sustained damage, so always position to collect them before re-engaging.
