Pre-6, you lose this lane.
Aphelios has flexible weapon cycling that lets him poke or burst depending on the situation, Milio's shield absorbs your Q-stacked autos, and you have no mobility without R — Milio's W root on you while Aphelios swaps to Crescendum is a kill threat you cannot escape.
Post-6, your R fundamentally shifts the 2v2 in your favor if you activate it proactively: your W laser with 50% reduced CD and E dash let you stick to Aphelios through Milio's peel and your sustained Q splash damage will outscale his burst over 15 seconds.

Your best trading pattern is to use W to slow Aphelios from range and force Milio to burn his W root reactively, then activate R when both their defensive cooldowns are spent and dash E onto Aphelios to deliver full Q-stacked splash autos with Senna's CC chaining.
Never engage without Senna's root available — Milio's E speed boost lets Aphelios disengage faster than you can chase without your R dash.

Push the wave with your Q active splash damage to deny Aphelios safe farming and force Milio to choose between shielding chip damage or roaming.
You want a slow push into a freeze near your tower when possible — this keeps you close to your tower during Aphelios's dangerous weapon windows and lets you stack Q passively while Senna pokes them down.

Do not activate R reactively when you are already low or when Milio's W root is available — your R is your escape and your win condition, and burning it defensively means you have no mobility for 90+ seconds while Aphelios can freely cycle into his best weapon against you.
Stacking Q on minions instead of looking for poke opportunities on Aphelios before your R is a passive mistake that lets Milio sustain through your damage and denies you the lane pressure you need.
