Who Wins

Pre-6, Swain wins most extended trades because his W root into E drain combo deals more burst damage than your Q sustain can offset, and his passive soul fragments give him stacking HP that compounds over time; you lose 1v1 if you let him land W.
Post-6, you gain a meaningful power spike with R that lets you survive his full combo and potentially out-sustain him in extended fights, but Swain's R also activates at 6 and his drain becomes nearly unkillable in a prolonged duel, so you rarely win a straight 1v1 even post-6.

Your best pattern is to Q-pool (W) through his W root to deny the setup for his E drain, then re-engage with Q once his E is on cooldown; short trades where you land Q and immediately pool his follow-up are your only favorable exchanges.
Never stand still long enough for him to root you, because a rooted Vladimir without pool available is simply losing a trade against Swain's E sustain.

Push the wave consistently with Q harass to force Swain to choose between farming under tower and walking up to trade; Swain is a strong all-in champion but a weaker wave-clear champion early, so denying him free farm under pressure is more valuable than trying to kill him.
Shoving also lets you roam or reset safely, which is more productive than attempting a kill in a lane where you are not the 1v1 favorite.

Do not use W (pool) preemptively or wastefully — it is your only tool to dodge his W root, and if you burn it for poke avoidance rather than saving it for his all-in setup, you will die to his full combo with no escape.
Burning pool on a non-threatening situation is the single biggest mistake you can make in this matchup.
