Pre-6, you lose the lane poke war because Milio's E shield and passive healing directly counter your W heal-trade pattern, and Caitlyn's range keeps you from safely applying your E.
Post-6, Milio's R is a disengage tool that can neutralize your W setup, so you only win post-6 if you land a clean W into an all-in before Milio can react.

Your best pattern is to bait out Milio's E on a poke, then immediately W onto Caitlyn to force a real trade while she is unshielded — your E on Aphelios during this window amplifies his burst significantly.
Do not use W as a heal reactively; use it offensively to initiate the trade sequence.

You should not roam early because Aphelios is immobile and leaving him alone against Caitlyn with Milio is a death sentence — your presence in lane is mandatory.
Only consider a roam post-6 if the wave is fully pushed in and Aphelios has enough items to survive a short window alone, targeting mid where your W can secure kills.

Do not spam E on Aphelios for minor poke trades — Milio's healing will recover the damage and you will run out of mana, leaving you unable to sustain or engage when a real opportunity appears.
