Pre-6, you have a meaningful advantage at level 1 and 2 through Q poke and passive on-hit damage from range before Mordekaiser has reliable tools to close the gap, but once he hits level 3 and has E, he can pull you into his passive aura and the trade flips heavily against you if you're not at full Q stacks ready to cash in.
Post-6, you lose the 1v1 if you get caught in R — your E is stripped away, your W heal is limited, and Mordekaiser's stat steal combined with his passive aura will outlast your burst unless you've built a significant health lead going in.

The correct trade pattern is to stack Q to 3 marks from max range, then use E to blink in, consume the stacks with your empowered attack, and immediately recast W to heal back before Mordekaiser can fully stack his passive — the key is that you initiate and disengage on your terms, never letting him dictate the engagement duration.
If you let the fight extend beyond your initial burst window, his passive aura and W shield-into-heal sustain will out-value your damage.

Keep the wave slightly frozen or slow-pushed near your side to maintain the range advantage for Q stacking, as Mordekaiser wants to shove and close distance — a wave near your tower forces him to walk into your Q poke to CS and punishes his passive-stacking attempts.
Avoid hard shoving into his tower, which gives him a free reset position and lets him walk up to you without minion interference.

Do not activate W as a panic button when Mordekaiser uses R on you, because burning W early inside the Death Realm means you heal for less and lose your movement speed steroid for repositioning — instead, try to force his R when your W is already on cooldown so you can plan around it, or save W specifically to maximize the heal by taking as much damage as possible before recasting.
Walking into his R without an escape plan or health advantage is a death sentence regardless of your combo.
