Who Wins

Pre-6, you have a real level 1–2 window where your Q-empowered autos outrange his melee attacks and your W shield absorbs his passive aura damage before he can stack it fully; however, at level 3 Mordekaiser's E pull removes your ability to disengage and his passive proc will outdamage your burst if the fight extends.
Post-6, you lose the straight 1v1 because his R strips you of your entire kit's utility — your E wall-stun, your R suppression, and jungle assistance all become irrelevant inside Death Realm while he steals your stats and burns you with passive aura, so avoid giving him a clean R opportunity at all costs.

Your best trading pattern is to use Q to empower autos and poke him down from the edge of his E range, then commit only when his E is on cooldown by using W to shield his passive aura, walking in for Q-empowered autos to stack your passive Quaking for max HP damage, and saving E to either disengage through terrain or pin him against a wall for the stun.
Never let the trade go long enough for his W healing to kick in — hit your Quaking proc, deal the max HP damage burst, and back off before his sustained aura and healing swing the fight.

Play for a slow push into a freeze near your tower to force Mordekaiser to overextend for CS, which creates wall-collision opportunities for your E stun since the tower and lane boundaries give you reliable terrain to work with.
Avoid hard shoving because a pushed wave gives Mordekaiser a safe environment to R you with no minion interference and no jungle path to reach you in time, which is the exact scenario you cannot survive.

Do not use your R offensively on Mordekaiser in a side lane 1v1 — it suppresses him briefly but he will immediately R you in response, and inside Death Realm your suppression has already ended while his 7-second stat-steal fight is just beginning.
Never engage him with low Quaking stacks and no W shield available, as without the max HP damage proc and the damage absorption you have no way to burst him down before his passive aura and W healing take over the fight.
