Xerath massively outranges you pre-6 and will poke you down with his Q and E before you can ever close the gap to stack your Q marks — you lose the 1v1 decisively before level 6, and your only meaningful spike is level 3 when you have E to close distance, but Xerath's root (E) will punish any telegraphed engage attempt.
Post-6 your R gives you execution pressure and a 99% slow to set up your combo, but Xerath's R is a global-range poke tool that can keep him safe and topped off on distance, so you only win post-6 if you can land your full combo on a Xerath who is already low or caught without his E root available.

Your best trading pattern is to use Q to stack marks from max range while walking up incrementally, then wait for Xerath to misposition or waste his E root before committing your E blink into W to cash in all stacks — the combo only works if you enter with 3 Q stacks already applied, because without them your burst is too low to threaten a kill.
Never activate W before blinking in, as the true damage drain will kill you if you're kited; instead, blink first, auto to consume stacks, then toggle W to sustain through his counterplay.

You need to shove the wave fast and look for roams, because sitting in a poke lane against Xerath drains your health bar without giving you meaningful combat windows — use your E reset potential on minions to clear efficiently and create tempo advantages through the map rather than trying to win the lane directly.
When the wave is frozen near Xerath's tower, do not force fights into his optimal range; instead, stack Q on the wave to maintain marks and wait for jungle pressure to create a 2v1 scenario where your burst becomes lethal.

Do not activate your E blink into Xerath when he has his E root available and you are at full health — he will root you mid-combo, you will take his full burst while your W drains you, and you will die without completing the kill.
Patience is mandatory in this matchup; a premature all-in without Q stacks pre-applied or without Xerath's root on cooldown is almost always a losing trade given his range advantage.
